Relief for those affected by Hurricane Florence

Origin SC was recently awarded limited funds to help those affected in South Carolina by recent hurricanes and flooding. We have made the following application for those looking to receive assistance. Once you click the below link for the application, save the PDF to your computer to continue to electronically fill out for submission.

The application itself takes approximately 10 minutes to fill out, and another 10-15 minutes to gather the required supporting documents.

Instructions to create a Digital ID to electronically Sign:

  1. Double click the signature block
  2. Select “Configure Digital ID”
  3. Select “Create a new Digital ID”
  4. Choose “Save to File”
  5. Fill in your name, email and click “Continue”
  6. Create a password (minimum of 6-digits) and click “Save” then “Continue”
  7. Fill in your password and click “Sign”

Once set-up, each time you click on a signature block, a pop-up with the information you added above will appear. Enter your password and hit “sign”. You will be prompted to save the document with each signature.

Once you are finished, you have several options to submit the information to us, please do not email as this is not a secure method of communication.
